Tetbury Wellbeing will collect and store Personal Data to include such information as name, contact details, date of birth and relevant medical history to ensure you are provided with the very best tailored, safe and effective treatment. Any Personal Data collected may be shared with Tetbury Wellbeing Practitioners (where relevant) but we will not share your Personal Data with third parties. All emails and texts will be deleted as soon as is practical.

Personal Data (Consultation notes) will be stored on paper in locked cabinets, for 7 years after the last occasion on which a treatment was carried out, or for a minor, 7 years after they reach the age of maturity (18) which complies with The Statute of Limitation. By signing the Consultation form, you are Consenting to your Personal Data being collected and stored for the specific purpose of a treatment being carried out, to comply with the Legal Obligation of our Insurance Company. As a Data Subject, you have the right to access Personal Data that we may hold on you, which we will provide within a month. If you are unhappy with the way in which your Personal Data is being collected and stored, you have the right to complain to the Information Commissioners Office (ICO) with whom we are registered.
